Memberships and Packages

 All Packages: IV treatments, procedures and or prepaid packages are non-refundable. Packages are non-transferable and must be used within one year or specified package time frame from the date of purchase unless stated otherwise in the packages. (Example 4-week package expires at end of 4 weeks, 12-week packages expire at the end of 12 weeks) Sessions not used will be forfeited and not refunded.    

Memberships:

1. Minimum Term and Legally Binding Agreement: The Member agrees to a minimum term of twelve (12) months from the date of signing this Agreement. This Agreement is legally binding and constitutes a commitment by the Member to comply with the terms and conditions set forth herein.

2. Monthly Auto-Draft of Payment: The Member agrees to monthly auto-draft payments for the duration of this Agreement with 12 month minimum required. Payments will be automatically deducted from the Member's designated account on a monthly basis unless the individual chooses to receive the 10% discount for paying for 12 months membership at signing, then monthly draft will start on the 1st month. First payment is the day of purchase then monthly on the 1st day of the month. 

3. 10 % Discount for Full Payment: The Member is eligible for a ten percent (10%) discount on the total membership fee if the full amount for the twelve-month term is paid in full at the time of signing/purchase of this Agreement. The Member must notify staff prior to signing/purchase if they wish to take advantage of this discount. 

4. Booking and Attendance Policy: If a session is booked by the Member and not attended, the session will not be refunded. The missed session will still be deducted from the monthly session allowance included in the Member's package.

5. Discounts for Purchases: Discounts included in various memberships only apply to future purchases.

6. Transferability of Membership: The membership cannot be transferred to another individual without prior approval after a formal written request.

7. Sharing Membership Sessions: For an additional fee of $50 per additional person, the membership sessions can be shared that month with the specifically named individual.

8. Upgrading Memberships: Memberships cannot be canceled prior to the twelve-month term. However, memberships can be upgraded with a new signing date, which will adjust the term date accordingly.

CANCELLATIONS: We understand that emergencies do arise; however, we request at least 24-hour notice for rescheduling or canceling all appointments. Failure to do so may result in your account being charged. NO SHOWS WILL BE BILLED FOR THE SCHEDULED IV TREATMENT.

Note Additionally, any unused sessions remaining on your monthly membership will not roll over to the next month. Sessions must be used within the month they are allotted, as they will start fresh each month. If you don't use them, you lose them. 

Please be advised that all memberships are subject to a minimum term of 12 months in purchasing membership you agree to 12-month commitment monthly auto draft payment unless 12-month membership fees paid in full at time of purchase or prior to first session.  

Membership Cancelations:  To cancel after the Minimum Term has expired (12 months), you can either email a notice of cancellation to AwakenAlternativesLTD@gmail.com or complete an Awaken Alternatives' standard “Cancellation Notice” signed and submit it in person to an Awaken Alternatives employee. If you choose to cancel by email, please ensure that your email includes your full name, phone number on file, and a request to terminate your Membership. The standard form of cancellation notice must be received at least thirty (30) days prior to the effective cancellation date. Failure to comply with these cancellation terms will not result in the cancellation of your Membership, and you will continue to be charged as per the terms of this Agreement until a valid cancellation is submitted. Note that canceling your Membership during the Minimum Term is not permitted, except as otherwise specified below: If you become physically incapable of receiving a significant portion of the services outlined in this Membership Agreement or if you pass away, you or your estate may terminate this Agreement at any time. Awaken Alternatives may request proof of your disability or death before terminating this Agreement. Upon valid cancellation, Awaken Alternatives will refund any unused portion of your Monthly Fee within thirty (30) days of receiving your Cancellation Notice. You have the right to cancel this Agreement within three days of receiving an executed copy, up until midnight of the third day. If you cancel during this period, your Membership will be terminated but you will not receive a full refund of the Monthly Fee. In the event that the third day falls on a federal holiday or when the Awaken Alternatives location where you signed up is closed and you cannot access their facilities or services, you may notify Awaken Alternatives of your cancellation by sending an email to AwakenAlternativesLTD@gmail.com or calling the office and leaving a detailed message.
